Community Outreach Nurse

Summary

The Community Outreach Nurse is responsible for overall management of the medical aspects of the AFC and MassHealth programs, including the direct oversight and supervision of staff. The nurse is responsible for administration of the medical/nursing components of the AFC program. S/he is responsible for ensuring programmatic and contractual obligations are met. S/he implements the organization’s mission and values by supporting individuals to make positive changes in their lives.

Up to 25 hours/week, including evenings and weekends and overnights
Schedule based on programmatic need

Required Skills

The ideal candidate must possess excellent organizational, communication, and creative abilities. The demonstrated ability to work independently in a team environment is required, as is flexibility to meet program needs.

Education, Training, and Experience

Associate’s degree with R.N. license and five years related experience OR Bachelor’s degree with R.N. license and three years related experience. One year of supervisory or human service experience is desired.

Other Requirements

Must have a valid driver’s license, three years driving experience, acceptable driving record, the use of a vehicle, and the willingness to transport program participants in a personal and/or agency vehicle. Desire to do meaningful work and make a difference in someone’s life a must. Bilingual/Bicultural experience is an asset.
